The world survives; the people at the center of the scene do not walk away.
What happens in Ending C?

What is the Old God?
The ending reframes the map itself. Endacopia is not simply a collection of disconnected dream locations. The house, Misery Town, Timesville, and the Office exist within a vast dying being that the Surgeon treats as God. The crushing walls and failing regions are symptoms of that host reaching the end of its life.
That reveal also explains the Surgeon's unusual mix of medicine and devotion. He is not only repairing individual bodies. His final project is surgery at the scale of the entire world. The procedure succeeds: the heartbeat resumes, and the giant body moves again.
Does Mellow die in Ending C?
The strongest reading is yes. The ritual requires Mellow and does not show him leaving afterward. The closing images present revival of the host, not rescue of the participants. The game leaves room for metaphysical questions about identity and what continues inside the revived being, but it gives no ordinary survival scene for Mellow.
Do the Surgeon and the Bride die?
The scene treats the Surgeon and the Bride as the other required components of the same procedure. Like Mellow, neither receives an escape afterward. This is why the ending feels tragic even though the Surgeon's plan works: he achieves the purpose that has directed his long life, then becomes part of its cost.

The safest description is the lore ending: Ending C explains more of the world's machinery and the Surgeon's motive than A or B. Whether that makes it the best, worst, or most truthful ending is part of the game's unresolved moral choice.
